Version History
- Added REFERENCE.md for detailed API usage, examples, tool descriptions, polling, and embed info. - SKILL.md is now a concise summary and directs users to REFERENCE.md for full documentation. - Clarified REST and MCP server invocation and configuration. - Streamlined endpoint, request, and response descriptions. - Documented rate limits, data retention, and polling recommendations. - Added schema field types, validation, and embed widget usage.
What This Skill Does
Collects structured human input during automated workflows via web forms. Define a form schema, distribute unique URLs to recipients, and poll for responses before the workflow continues. Supports approvals, decisions, reviews, and arbitrary data collection.
Returns machine-readable structured responses an agent can act on directly, instead of unstructured replies scattered across email or chat.
When to use it
- Requesting manager approval before deploying a budget change
- Gathering structured feedback from multiple reviewers on a design
- Collecting employee responses to a policy update before rollout
- Getting sign-off from stakeholders on a contract draft
- Running a post-incident review with required response fields
Example Workflow
Here's how your AI assistant might use this skill in practice.
User asks: Get approval from 3 managers for a Q3 budget increase before proceeding
- 1Creates a form with a radio decision field and optional notes textarea via POST /api/v1/forms with recipient_count: 3
- 2Receives 3 unique recipient URLs from the API response
- 3Distributes the URLs to the managers via email or Slack
- 4Sets up a cron job to poll the form summary every 10 minutes until submitted_total equals known_total
- 5Fetches results via GET /api/v1/forms/{form_token}/results and passes structured responses to the next workflow step
JSON array with each manager's decision (approve/reject) and notes, consumed directly by the workflow
